What is a PCMH?
PCMH stands for Patient Centered Medical Home. Our Inspira Medical Group primary care practices embrace a team-based approach, led by your doctor, to make it easier for you to access the care, information and assistance you need, when you need it.  Using your PCMH as your first point of contact allows you to receive more coordinated, personalized health care.  You can expect:

Quality, team-based care led by your doctor that’s personalized, friendly and responsive

  • Coordinated care through your doctor, from start to finish
  • Access to care you need, when you need it
  • Enhanced communications with your care team by phone, e-mail and in person
  • Wellness and preventive care based on national guidelines\
  • Extra support and resources to protect your health- including health education
Why We’re Making This Change
 Even though we have not completed PCMH certification, when you visit us today, you’ll find that many of these changes are already in place or underway at Inspira Medical Group.
 Why does this approach to medical care matter? Because research tells us that in countries where medicine has long been practiced in this way, patients experience much better results. Patients:

Patient Centered Medical Home Responsibilities
The health and wellness of our patients is our primary focus at Inspira Medical Group.  Providing outstanding care to our patients is our top priority and goal.  To achieve this goal, the care team and patients must work together.  This working together concept is the foundation for the Patient Centered Medical Home.

As your Care Team, we pledge to:

  • Learn all about you, your history, your care preferences and your health goals.
  • Explain diseases, treatments and results in an easy-to-understand way.
  • Keep treatments, discussions, and records private.
  • Be available to your during and after regular business hours for your care needs.
  • Provide instructions on how to meet your healthcare needs after hours.
  • Give you clear directions about treatments and medications.
  • Listen to your questions and concerns to help you make decisions about your care.
  • Coordinate your care with trusted experts if needed.
  • Be sure to communicate:
    • Test results in a timely fashion
    • Information about alternative services to help you maintain a healthy lifestyle
    • Updates on vaccines and screenings
  • End every visit with clear instructions about expectations, treatments and future plans.
We trust you, as our patient, to:
  • Know that you are a full partner in your care. 
  • Ask questions and share your feelings.
  • Be honest about your history, symptoms and other important information
  • Prepare for and keep scheduled appointments or re-schedule as early as possible.
  • Let us know when you have seen another doctor or received other healthcare services so we can be sure we are all working together for you.
  • Make healthy decisions about your daily habits and lifestyle.
  • Follow the plan you and your doctor agreed upon, including taking medications as directed.
  • Call your doctor first, with all problems, unless it is a medical emergency.
  • Give us feedback to help us improve our care for you.
Continuity of Care
At the heart of the PCMH model is the relationship between a patient and a provider and his/her practice team. Seeing the same provider has been shown to have a positive impact on your health. Continuity of care is a process in which the patient and the provider work together to develop a plan to manage the ongoing healthcare needs of the patient. At Inspira Medical Group we ask you to pick one primary provider that you will see as much as possible, allowing that provider to get to know you and provide you with the best care possible. 

How will I contact my Care Team?
You can contact your Medical Home Team directly to arrange an appointment or to discuss your healthcare needs by calling the office during normal office hours. Outside of the normal office hours, please call the office and the answering service will contact an on-call provider. 

Patient Services
We know your life is full of ups, downs, twists and turns. That’s why our comprehensive healthcare covers unexpected illnesses, disease management and routine check-ups.  Please check each office’s page for their specific list of services. 

Urgent Care Centers
When your life gets busy, sometimes you need access to quality care that doesn't take even more time away from work or your family. That's where Inspira Urgent Care comes in. If your doctor is not available or your minor injury or illness does not require the life-saving services of a hospital emergency room, simply walk right in! No appointment is necessary.

The urgent care center is not a replacement for the emergency room. Emergency rooms are staffed 24 hours a day and are designed to treat patients suffering from life threatening injuries or serious illnesses.

Emergency Room
If you have a serious condition (stroke, heart attack, severe bleeding, head injury or other major trauma) go straight to the nearest ER. Don't take a chance with anything life-threatening. The ER is the best place for these and other critical conditions, including: 

  • Chest pain
  • Difficulty breathing
  • Severe bleeding or head trauma
  • Loss of consciousness
  • Sudden loss of vision or blurred vision
Please call 911 for a true emergency. Please have a family member call us to let us know you have gone to the hospital. If able we will forward health information to aid in your care, as well as be better able to coordinate your follow up care. If admitted to an area hospital, nursing home or rehabilitation center, please call or have a family member call within two business days of discharge to set up a follow up appointment.
